GEMS Pvt. Ltd. has developed a
proprietary therapeutic system -Thermo Ablator System® (TAS®),
as a less invasive treatment for women who suffer from excessive
menstrual bleeding or menorrhagia, due to benign causes and for
whom childbearing is complete. The GEMS TAS System is a safe,
low cost and effective treatment that can be performed in a variety
of clinical settings, including small nursing homes. |
Menorrhagia is a common complaint
in women of post childbearing age. It is quite distressing and
inabilating especially in modern times, as many women are workingwoman
apart from being housewives. |
Menorrhagia causes distress.
Additionally it debilitates by giving rise to Anemia and other
satellite effects associated with it. Detailed investigations
revealed that 40% of patients have hormonal imbalance but uterus
and other organs are otherwise normal i. e. no evidence of malignancies
is found. |
| In properly screened and select
cases thermoablation of endometrium is the most preferred treatment.
It is a simple procedure carried out under SA or GA in a day -
care system. |
| The newly designed Thermoablator
machine uses a catheter with balloon to be introduced through
cervix. Plain Water at temperature between 75 C and 92 C is circulated
at uterine pressure of 200 mm Hg. |
Depending on set temperature
value, the ablation time will vary. Lower the temperature more
will be the time required. At mean Temperature of 87 C it takes
12 to 15 minutes, while at 92 C, 7 to 8 minutes approximately
will yield the desired results. This ensures Complete Burning
of up to 7 mm thick endometrium. No Curettage or preoperative
medication is required- (As in Other systems, thereby increasing
medical expenses burden on patient) |
This procedure has a distinct
advantage. It is inherently safer. Also the heating medium is
not directly in contact with endometrium. This results in better
procedure management. It also reduces cleaning needs after the
procedure compared to direct contact method employed in other
methods. |
Balloon, when distended, contacts
all surface area and is capable of covering over 98% of the corners
and contours of the uterine cavity. As a result, unablated surface
patches are nearly eliminated even at the cornual ends. |
Endoscopic Surgeons in the United
Kingdom have used pressures of 30 to 50 mm of Hg for ablation
in direct method without balloon and have obtained good results.
However this method has several disadvantages compared to our
system. |
Majority of women report hypomenorrhoea
or total amenorrhoea after just one seating with thermoablation.
Even if total amenorrhoea is not achieved satisfaction rates are
good. |
It should be clearly understood
and highlighted that thermal ablation is not an option for hystrectomy
but the indication in itself. As previously untreatable menorrhogeis
had perforce to be treated with surgical extirpation of uterus.
Hence in absence of uterine pathology thermal ablation is the
treatment of choice. |
| Lastly costs etc. are also of paramount importance.
They are in terms of money, hospitalization, time and non working
days days. Thermal ablations being day-care procedure, all these
aspects are in its favour. |
